You can either use the "GO" button or the little germ looking button to the right of "GO".  When you click on the germ button you can choose to either spend your starting points to upgrade your virus or hold on to them.  Either way, hit the "GO" button until people start dying.  Watch as your points increase.  Upgrade your virus.  Kill indiscriminately.
